This policy describes how the Restorative Wellness website handles information. How we use and protect your health information as a patient is described in our Notice of Privacy Practices.
Information you choose to provide
If you request an evaluation, you provide your name, phone number, a brief description of the reason for your request, and, if you choose, your email address, preferred callback time and voicemail permission. This information is used only to contact you about a potential evaluation.
Evaluation requests submitted through the website are handled through Google Workspace and Google Apps Script and stored in restricted-access Google Workspace systems used by Restorative Wellness. Submitted information may include your contact information and the brief health-related information you choose to provide. Access is limited to authorized practice personnel. Restorative Wellness uses Google Workspace subject to its applicable business associate agreement for this workflow.
Information collected automatically
When you visit the site, the hosting service necessarily receives technical request data such as your IP address, browser information, requested page, date and time so it can deliver and secure the site. The site does not include a separate analytics service. Information entered in the evaluation-request form is not added to Restorative Wellness website URLs or sent to analytics.
What we do not do
- We do not sell your information.
- We do not use advertising pixels, session replay or heatmap tools.
- We do not use artificial intelligence to process health information you submit.
Cookies and browser storage
The site does not store evaluation-form values in cookies, local storage or session storage. Its navigation framework may use session storage only to restore scroll position while you browse. The hosting platform may also use technical security or delivery mechanisms outside the site's application code. This policy should be rechecked if site services change.
Please limit what you share
Please share only what is needed to understand the reason for your call. Do not send health details by ordinary email or text.
Changes
We may update this policy. The effective date above reflects the latest version.
